The following items are required in order to submit the form: A valid email address (NoEmail.animalrides@gmail.com - for those who don't have an email address) The customer's First Name (Guardian for young children) A valid phone number (In case of Overtime or Incident) When a customer asks why they have to fill out the waiver, simply tell them it's 'just for liability and so they know to follow the riding rules.'
